Introduction

Introduction to Eyelid Surgery

Drooping eyelids, persistent under-eye bags, and excess skin around the eyes can make you look tired or older, or even impair your vision. These common concerns affect both men and women, often due to factors such as aging, genetics, or lifestyle choices. While skincare and cosmetic treatments can offer temporary improvements, they can’t correct sagging skin or restore a refreshed, youthful appearance the way eyelid surgery can.

At Lugo Plastic Surgery in Athens, GA, board-certified plastic surgeon Dr. Liana Lugo specializes in upper and lower blepharoplasty, a simple yet transformative procedure designed to rejuvenate your eyes. Whether you’re struggling with drooping upper lids, puffy under-eye bags, or obstructed vision, eyelid surgery can help you achieve a brighter, more youthful, and well-rested look—all with minimal downtime and long-lasting results.

Upper Blepharoplasty

What is
Blepharoplasty
(Eyelid-Surgery)?

Blepharoplasty is a surgical procedure that removes excess skin and fat from the eyelids to rejuvenate their appearance. It can be performed on the upper and/or lower eyelids to improve the appearance of sagging, drooping, or puffy eyelids.

  • As we age, our eyelids can begin to sag and droop, giving us a tired or even haggard appearance. Upper blepharoplasty, also known as an eyelid lift, is a surgical procedure that helps correct signs of aging.

During an upper blepharoplasty, Dr. Lugo will make incisions along the natural crease lines of your eyelids. She will then remove excess skin and fat from the eyelids. This can open up your eyes and give you a more youthful appearance.

Upper blepharoplasty is a relatively common procedure that is performed under local anesthesia. It typically takes about one to two hours to complete. Most patients experience some bruising and swelling after surgery, but this usually resolves within a few weeks.

If you are considering upper blepharoplasty in Georgia, Dr. Lugo can help you determine if you are a good candidate for the procedure. She will discuss your goals and expectations and create a treatment plan tailored to your needs.

  • A lower blepharoplasty reduces under-eye bags and puffiness that can form below your eyes for a multitude of reasons. The lower eyelid can present several aesthetic issues, so Dr. Lugo will tailor the approach to address the patient’s specific concerns. The incision is usually just below the lash line or inside the lower eyelid.  Dr. Lugo will then reposition the muscles and remove fat from the under-eye area. She will also tighten loose skin. The procedure varies from patient to patient, depending on their preferences and the complexity of the concern. The type of anesthesia used will depend on your preferences, suitability, and the preferred surgical technique.

The surgery technique will depend on the concerns. For a lower blepharoplasty, Dr. Lugo will usually make an incision along the lash line and within the crease of the upper eyelid for an upper blepharoplasty. She will remove excess skin and reposition the fat and tissues.

To treat the lower eyelid, an incision can be made either on the inside of the eyelid (transconjunctival) or on the outside (transcutaneous). The advantage of the inside incision is that it minimizes some of the risks, and the scar is better concealed; however, if there is excess skin, this cannot be treated. The outside incision provides access to any of the eyelid’s structures, but it results in an external scar. Through either incision, the fat pads of the eyelids can be removed or repositioned to treat bags and provide a more youthful, smooth transition from the lower eyelid to the cheek.  Although both upper blepharoplasty and transcutaneous lower blepharoplasty result in scars, these usually heal very well. When the incision is placed on the inside of the lower eyelid, the scar is invisible.

What Are the Benefits of Eyelid Surgery?

Benefits of Upper Blepharoplasty include:

  • Improved appearance: Upper blepharoplasty can help to reduce the appearance of sagging eyelids. This can give you a more youthful and refreshed appearance.
  • Improved vision: In some cases, upper blepharoplasty can enhance vision by removing excess skin that blocks the visual field.
  • Increased self-confidence: Many people who undergo upper blepharoplasty report feeling more confident and self-assured after the procedure.

Benefits of Lower Blepharoplasty include:

  • Removes or repositions fat deposits for a smoother, more youthful appearance.
  • Tightens or removes excess skin to minimize wrinkles and sagging.
  • Eliminates a tired or aged appearance by improving under-eye hollows and puffiness.
  • Unlike fillers, lower blepharoplasty provides lasting improvement.
  • Incisions are typically hidden along the lash line or inside the eyelid, making scars virtually invisible.

Who Are the Best Candidates for Eyelid Surgery?

Good candidates for blepharoplasty are individuals who have excess skin or fat on their eyelids, causing a tired or aged appearance. Ideal candidates should be in good health, have realistic expectations, and not have any eye conditions that could affect healing. In summary, this includes:

  • Patients with realistic expectations
  • Patients looking for more rested, youthful-looking eyes
  • Patients with sagging or excess skin on the upper eyelids
  • Patients who have under-eye bags or fluid accumulation under the eyes
  • Patients with vision affected by sagging upper eyelids
  • Patients who have unwanted creases around the eyes

What’s the
first step?

Your first step in the process of getting eyelid surgery is to consult with an experienced plastic surgeon. In Athens, Dr. Lugo works with patients through virtual or in-person appointments to develop a surgical plan tailored to their individual needs. She will assess the anatomy of the eyes, listen to your concerns and ultimate goals, and review your medical profile to ensure this procedure is safe for you. Once this evaluation is complete, she can develop your surgical plan, including the areas to be addressed, the techniques to be used, and what you can expect before and after surgery.

Once your surgery is scheduled, you’ll be given your pre-operative instructions. You’ll also schedule your pre-surgical appointment a few weeks prior. Please don’t hesitate to ask any questions or express any concerns you may have, as this is your opportunity to become informed and comfortable.

What to Expect on the Day of Eyelid Surgery

You’ll arrive at the Athens surgical facility, where Dr. Lugo will review the procedure and mark incision areas. Depending on the extent of the surgery, you’ll receive either local anesthesia with sedation or general anesthesia for comfort. The procedure typically takes one to two hours. For upper eyelid surgery, excess skin and fat are removed through an incision in the natural crease, while for lower eyelid surgery, incisions are made along the lash line or inside the eyelid to address puffiness and sagging.

After surgery, you’ll spend time in a recovery area until you’re fully awake. Mild swelling, bruising, and temporary blurred vision are common but manageable with cold compresses and prescribed ointments. Since you won’t be able to drive, a friend or family member should take you home.

Recovery

Keeping your head elevated and using cold compresses helps minimize discomfort, and any mild pain can be managed with medication. Stitches, if placed, are usually removed within five to seven days, and most patients can return to work within a week if their job isn’t physically demanding.

Residual swelling diminishes within two to four weeks, and the eyes appear brighter and more youthful. Makeup can typically be worn after two weeks; sun protection is essential to prevent pigmentation in healing skin. While full results take several months to finalize, the improvements are long-lasting, often lasting a decade or more.
